Registration for the 2012 Football and Cheer season is open! Below is a list of the upcoming registration dates and times. Teams will fill up fast so don’t delay!

Saturday, May 19th – Red Bug Lake Park (Pavilion B)- 9:00 a.m. to 1:00 p.m.

Saturday, June 23rd – Red Bug Lake Park (Front Area by Basketball Courts)- 9:00 a.m. to 1:00 p.m.

Documents needed:

– Two (2) Copies of Completed 2012 Registration Form*
– A physical dated after January 1, 2012
– Signed Participant/Player Contract*
– Birth Certificate: Original or Certified copy of plus an additional copy for our records
– Driver License, Passport, or Florida ID showing current address of Parent or Guardian
– Copy of Final Report Card for 2012 (this will be collected at the end of the school year)
– Registration Fee** (cash or check, Please make check payable to ‘LHPWLS’)
– Football: $200
– Cheerleading: $285

** $50 non-refundable deposit is acceptable but registration in full is preferred.

The 2011 Pop Warner Season Begins August 1st!

Lake Howell will be collecting last minute paperwork from 5:00 – 5:30PM. Please have all paperwork handed in by this time in order for your son or daughter to be able to practice at 6PM.

• Time: 6:00 – 8:30PM (please arrive early)

2011 Practice Field Assignments

For those that are new to the association please review to know where you need to go tomorrow.

All forms must be turned in prior to practicing. If your child is missing forms he/she will be pulled out of practice until they are turned in.

Please don’t put the board or coaches in an awkward position by asking them to make an exception. LHPW has been asking for all paperwork to be turned in prior to practicing since April!

Attire

(Please DO NOT wear pride packs to practice)

• Football: Helmet, chinstrap, mouthpiece, shorts, shirt, cleats
• Cheer: Towel, Shorts, shirt (no tank tops), hair pulled back and up.

LHPW will be holding a supplemental equipment pickup date on Saturday, July 30th at 9AM.

All registration fees must be paid prior to picking up your equipment. Please bring any remaining paperwork and fees to the equipment handout.

The location will be our storage unit at the facility immediate to the right of Gator’s Dockside in between Walgreen’s. Please proceed through the gate and make your first left hand turn.

CFP Physicians Group will be conducting physicals for those that still need them (which is most participants) on Saturday July 9th from 9:30AM until 12:00PM. Lake Howell Pop Warner has teamed up with CFP Physicians Group to offer “complete” physicals at a discounted rate.  Cost of the physical is $15 and CFP Group will be donating $3 of that back to our league.  Dates for the physicals are July 9th at a registration location to be announced. Registration fee for Football is $200, and Registration for Cheerleading is $200 with an additional $85 apparel fee for mandatory clothing items required for competitions and exhibitions.
