Registration for the 2012 Football and Cheer season is open! Below is a list of the upcoming registration dates and times. Teams will fill up fast so don’t delay!
Saturday, May 19th – Red Bug Lake Park (Pavilion B)- 9:00 a.m. to 1:00 p.m.
Saturday, June 23rd – Red Bug Lake Park (Front Area by Basketball Courts)- 9:00 a.m. to 1:00 p.m.
- Two (2) Copies of Completed 2012 Registration Form*
- A physical dated after January 1, 2012
- Signed Participant/Player Contract*
- Birth Certificate: Original or Certified copy of plus an additional copy for our records
- Driver License, Passport, or Florida ID showing current address of Parent or Guardian
- Copy of Final Report Card for 2012 (this will be collected at the end of the school year)
- Registration Fee** (cash or check, Please make check payable to ‘LHPWLS’)
- Football: $200
- Cheerleading: $285
* All forms can be found on the “Forms” page of this site.
** $50 non-refundable deposit is acceptable but registration in full is preferred.
